As a polysomnographer, I really do wish our service would do away w/ CPAP and change over to BiPAP. Folks with limited lung function, ect. cannot expell a constant inspitory pressure: interthoracic pressure is going to be a positive pressure throughout the breathing cycle. Once that patient meets the maximum volume threshold, that's it. There is the risk of retarding the thoracic pump w/ just CPAP. With BiPAP, there is two pressures: inspitory pressure is a higher level w/ a four percent decrease upon exhalation that way you do not exhaust the patient. The patients thoracic pump will excell. Better for the heart also...will be able to push the blood along.

Good morning everybody! My name is Marcia and I've been a FT/EMT with the Blanchardville Fire Dept of WI. It's been a little over two years now, and after going through the grueling schooling for this, I've come to the conclusion that there isn't much I can't handle. ;) I never would have thought that I would enjoy the medical field as much as I do, but now, I wouldn't change it for the world. Love to learn new things and am looking forward to meeting and getting to know other fellow EMS workers.

Hi all! My name is Crystal. I'm from Mukwonago, Wisconsin where I am a FT EMT w/ a private ambo service and a POC FF/EMT-I for Mukwonago FD. My husband is a fulltime FF/Medic at Big Bend-Vernon FD. Together, we have 4 beautiful children and love spending time just being together. I'm currently in school for my Associates in Psychology (graduate June '09!!) and then it's off to paramedic in the Fall. My hubby is in school for his Associates in Fire Science. I look forward to making/meeting some new friends in the field.

Hey there, my name is Julie and am originally from Eau Claire, but not live in Mondovi where I'm an EMT and have been for the last four years. I also work as a Polysomnographer at Sacred Heart Hospital in Eau Claire and have been here for four years now. I have four kids all from the ages of 8 to 17 and a husband who teaches in the UW system and has my full support...kids too!! Thinking about going back to school since my G.I.Bill will pay, whooohoo!! There are some pretty good topics here; I don't think there is a time where I haven't come away with learning something new or just a good refresher!! Keep 'em coming!

Don't mean to sound like a State EMS spokesman or anything but I also heard today that they are doing their "EMS Leadership Orientation" at 4 locations around the state in the next few months. I attended their pilot of this in Milwaukee earlier this year and thought it was very good. I encourage every service leader to attend. You will surely pick up something. It is an all day event but it is free.

Kevin, this is going to be a hot topic. Can it be moved to the discussion area?
I would love to see more about how this works in other areas. I think most all of us would agree that it is good for the patient in the long run but how do we get there? Do other areas do scenes and interfacility or just scenes? Interfacilities might be a little more difficult to achieve since hospital systems might have a little more interest in that.
How is it monitored so that all parties are playing fair? Does it work if one service doesn't want to participate? Is the WI Air Medical Council still active? Their last minutes posted were from 2007. They would certainly be a good place to start asking questions.

The State EMS Office just anounced dates and times for the "town hall meetings" on the EMS rules (110-113) undergoing revision. These rules pertain to all levels of EMS in Wisconsin. Don't miss an opportunity to have your voice heard. Check out the dates & times at http://dhs.wisconsin.gov/ems/
A copy of the rules will be posted there too although they are not there as of this moment.
Official public hearing will occur later.

How would you all respond to the idea of having 1 centralized dispatch center for all HEMS operations in the state. You call 1 number and the closest available helicopter for a scene or interfacility flight is contacted and dispatched.
