I like the term "unrecognized MI" better, since we don't know for sure the event was clinically silent.

Another management difference: STEMI vs NSTEMI. Many places emergently transport and cath STEMI patients, but take a wait and see approach to NSTEMI patients with elevated TNI and CK-MB. Additionally, transport services also take the same approach in treating the NSTEMI patient with less-agressive care than the STEMI patient...perhaps this a good place to look at preventative medicine, for example would more aggressive NTG administration help keep the latter patient from developing a STEMI? perhaps...
