Hello everyone!

   I am trying to plan an MCI drill for my department in June. We will have around 20-50 people participating (I don't have a good number yet) as providers and about 20 victims, give or take. I want to do something original - NOT A CAR ACCIDENT. We will mostly have EMS providers only but may have A fire department involved just for practice with IC. Does anyone have any good ones that you have done that were interesting? I live in Illinois and we have a lot of industry, water, schools, trains, airplanes, interstates.

 

   If you have any other ideas of things for me to avoid compromising the training, please let me know. Of  course it won't be perfect - MCIs never are, but what are some glitches that you have encountered that you would warn me against?

Based on a whole bunch of international data, the most likely terrorism scenario is bomb on a bus, which will produce 25 patients (4-10 serious). This makes for a manageable MCI scenario for almost any agency.

You're not really testing medical treatment skills here - you already know how to do this. You should be exercising ICS and triage skills, along with hospital destination coordination.

Since you have industry, look at doing an explosion of a pressurized vessel in a building. Vessel could have just been a steam tower or something. Make it where there were multiple patient's in the room at the time. I have also found that we practice on how to care for and treat yellow and red triaged patient's, but its the walking greens that can present a challenge. Coorindation with Police, Fire, EMS, Hospital, and Public works can really work well in situation like this.
I like the idea of a terrorist activity or perceived as such.... based on experiences of the Israelis you should have a few dead and dying on scene and everyone else fleeing the scene in search of their own medical assistance. See how the responders deal with no via able patients and many other patients showing up at the local ED and or doctors offices in the community.\
We ended up doing a bomb exploding at a high school. It worked out well, we managed with 18 victims. Unfortunately, all who were supposed to show for training did not, leaving us only our 6 shift people and 6 outside of that. But, it was an overall success. Thank you so much for all of the ideas, it gave me a lot to think about!
